1  All local cheques, bank drafts and telegraphic transfers should be made payable to “APCAACI 2010” in Singapore Dollars (SGD). Please note that a processing fee of SGD50.00 will need to be included for payment by Telegraphic Transfer or Bank Draft.
2 Please indicate the registrant’s name on the back of the local cheque or bank draft and mail to the Congress Secretariat.
3 Please indicate the registrant’s name on the front of the telegraphic transfer advice after payment has been made and either email, mail or fax the advice to the Congress Secretariat.

TERMS & CONDITIONS
• How to Register
  All participants are encouraged to visit www.apcaaci2010.org for individual online registration.
  Alternatively, you may complete and sign the PDF registration form and send it back, with the appropriate payment details and all related correspondence properly enclosed, to the Congress Secretariat at the address stated above. Kindly note that registration form received without payment or credit card authorisation will not be processed. Only individual registration will be accepted (one registration form per participant).
  For group registration (group of 10 delegates and more), please contact the Congress Secretariat for group registration details and instructions. All group registrations must be submitted by
Tuesday, 31st August 2010.
  If you have already registered online or faxed your registration form with all payment details, please do not mail a copy of the registration form as it may result in a duplicate registration.
• Registration Fees
All participants are required to pay the registration fee (in Singapore Dollars) stated in the registration form, which includes the following:
o Full Delegate / Trainee
   Name badge, congress bag, programme and abstract book.
   Admission to all scientific sessions at the 8th Asia Pacific Congress of Allergy, Asthma and Clinical Immunology, including workshops, debates, lunch symposia and exhibition hall.
   Invitation to Opening Ceremony and Welcome Reception.
   Two tea breaks per full-day Congress.
   Certificate of Attendance.
o Pre-Congress Courses 
   Name badge and Course notes.
   Two tea breaks per full-day Course.
• Registration Deadlines
o Deadline for Early Bird Registration : Wednesday, 30 June 2010 
o Deadline for Standard Registration : Friday, 15 October 2010
• Confirmation of Registration 
Once payment has been cleared, you will receive the confirmation either via email or fax only (confirmation will be mailed should email or fax details be unavailable). Please include your email address and fax number clearly in the registration form. To expedite the collection process, please bring along your registration confirmation letter to the registration counters (located at the Ballroom Foyer, Level 2, of the Congress venue) as proof of your payment and registration.

o One City Tour or a visit to the Underwater World at Sentosa.
o Singapore Shawl showcase at the exhibition hall.
o Entrance to the exhibition hall.
   
• Cancellation and Refund
Any cancellation and request for refunds must be submitted in writing to the “8th Asia Pacific Congress of Allergy, Asthma and Clinical Immunology” Congress Secretariat by Tuesday, 31 August 2010. Refunds, less SGD100.00 administrative fee, will be made within two months after the 8th Asia Pacific Congress of Allergy, Asthma and Clinical Immunology.
• Occupation
* Trainees: attach an official letter of certification from the Head of Department / Training director to qualify for the trainee registration fee.
** Nurses / Pharmacists: attach a copy of registrant’s nursing / pharmacist registration certificate for registration to the Allergy / Clinical Immunology Course for Allied Health Professionals.  
• On-Site Registration
For on-site registration, only cash (Singapore Dollars) and credit card payments will be accepted. The registration, tour and information counters will be located at the Ballroom Foyer, Level 2 of the Congress venue. Congress Dinner is payable at SGD150.00 per delegate or per accompanying person and it is optional. 
• Language
The official language of the congress is English. There will be no translation available.
